A 6-session evidence-based workshop designed for those dealing with a chronic or limiting health condition. Participants learn self-management techniques and skills needed in the day to day management of any type of ongoing health condition. The program has also been proven to be effective for caregivers.

If you have any long-term/chronic health condition that has taken over your life, or if you care for someone with a long-term/chronic health condition, the Living Well with Chronic Conditions class can help! Living Well with Chronic Conditions is a 6-week program geared towards helping people take back control of their lives. You will learn ways to manage the day-to-day symptoms that most chronic health conditions cause such as fatigue, difficult emotions, sleep changes, and pain. Learn how physical activity and nutrition can help and practice skills to improve understanding and mobility. Work together with other people, who understand what you are going through, to solve problems and achieve goals.
Live virtual class meets once a week for around 2 ½ hrs via Zoom. Thursday, March 11-April 15, 2021 3-5:30 PM.
This workshop will be hosted virtually online.
The program coordinator (or leader) for the workshop will reach out to all participants with instructions on how to access the workshop sessions.
